Humber River Hospital: Quality Improvement Plan


Humber River’s staff and physicians work hard every day to provide all of our patients with the same quality of care they would want for their own families. We continuously monitor a wide range of indicators to measure our success in that area, and to help us learn where more work may still be needed.

Our Quality Improvement Plan (QIP) provides a meaningful way for Humber River Hospital to clearly demonstrate our accountability to our patients and community. Our QIP is focused on creating a positive patient experience and delivering high quality health care. With this plan, we are able to very clearly see our targeted areas for improvement and chart our progress.

How to Read our Quality Improvement Plan

Our Quality Improvement Plan (QIP) is made up of two parts: 

  1. A document that provides a brief overview of our quality improvement plan, highlighting and listing our hospital’s top priorities for the year.
  2. A spreadsheet that includes our improvement targets and initiatives. The spreadsheet includes a core set of indicators that all similar hospitals across the province are working on.
The Health Quality Ontario has requested that all hospitals report on a series of core indicators to support province-wide comparisons. The core indicators that apply to our hospital are reflected in our QIP. Not all indicators apply to our hospital. In this case, you will see “N/A”.
