Humber’s Falls Prevention Program

Falls in seniors may result in serious injury and prevent them from participating in many daily activities. Fortunately, most falls are preventable and can be avoided through Humber River Health’s Falls Prevention Program.

About the program.
The Falls Prevention Program is an evidence-based program to improve senior’s safety. During the program, you will receive:
-a consultation with a geriatrician
-a falls assessment by a physiotherapist
-a group exercise program
-a home exercise program

Once completing the program, the team will provide ways in which you can continue to improve your balance, as well as resources you can connect with in the community.

Who should attend the program.
To attend this program, you must:
-be over the age of 65
-have mobility and balance difficulties
-be able to safely take part in a supervised exercise program
-be able to walk three metres
-have reliable transportation to attend the classes for 1.5 hours on Tuesday and Thursday mornings for six weeks

How to access the program.
To register for the program, a doctor’s referral is required. A geriatrician and a physiotherapist will then determine if the program is a correct fit for you.

The falls prevention program takes place in Humber River Health’s Healthy Living Clinic:

What to expect from the program.
During the program, you will attend:

1- An initial assessment.

Once you have completed a detailed assessment, you will be placed in a small group of six to eight participants to begin the program.

2- Sessions twice a week, for six weeks.

At each visit you will take part in:

  • an exercise class about one hour in length
  • an education session about 15 to 20 minutes in length, taught by various healthcare professionals

3- A discharge assessment and recommendations appointment.
