Why is the hospital collecting patient demographic information via this survey?

Our goal is to ensure that every patient has access to the highest quality of healthcare that we can provide.  The Ontario Human Rights Commission and Ontario’s Excellent Care for All Act (2010) encourage hospitals to collect and use demographic information to promote equity and track outcomes.  The information you provide will tell us who you are and help improve your patient experience and decisions about your health care.

What if there are question that I don’t want to answer?

Participating in the survey is completely voluntary.  You can refuse to complete the survey or skip any question that you want by marking the “Prefer not to answer” box.

Who can see my information?

We take your privacy very seriously.  Your answers will be securely stored in your health record and will only be visible to your health care team.  This information will be summarized and used by program leadership to help plan for services and provide service training.
