Welcome to Patient Connect: Humber’s New Appointment Reminder System

As part of our ongoing commitment to improving the patient experience, we are pleased to announce the launch of Patient Connect, Humber River Health’s new appointment reminder system.  

Patient Connect replaces our previous appointment reminder tools. With this new system, you will be able to: 

  • Receive reminders about your appointment by secure text messaging or automated phone call.  
  • Confirm your appointment. Rescheduling or cancellation require a phone call to the clinic. 
  • Enjoy a no-app, easy-to-use system that integrates with our scheduling tools. 
  • Text with administrative staff directly in real time, making asking questions regarding your appointments and faster and easier. 

Patient Connect is designed to be simple and accessible, with no app to download. It offers two-way messaging during practice hours for administrative support only.  

 No clinical information will be shared. Replies are typically provided within two business days. It also has features that can make it easier for caregivers to assist. 

Upon registration at Humber River Health, both new and existing patients are automatically enrolled to receive appointment reminders via text message. Participation is optional, and patients may opt out of text messages at any time by following the instructions provided in the reminder text. Patients who opt out of SMS will continue to receive automated phone calls for appointment reminders as part of our standard communication process.
